This was a health insurance provider.

They call twice a day for a week now.

Same thing in the email as above, be on alert and do not fall for this. It is an obvious attempt at fraud, IE they want your name number address and occupation. Also note that anything coming from a govt agency (as I work for one myself and was formerly a military man) will have either a .gov or .mil email adress
